Forms:
The following provides brief information on the various forms that are
used within Scouting. All of these forms can be obtained from the Scout
Information Center on 0845 3001818 or can be downloaded from Scoutbase
using the highlighted link within the description.
Leaders Forms:
Cancellation Form;
This pdf form should be filled in if a leader leaves the Group. It
should then be sent to the DC who will sign it and will pass it onto
the appointment secretary to update the District Records.
Adult Application Form; This pdf form is to be filled in for any new adult coming into the movement or for any change of appointment.
CRB Form; (Please note that these cannot be downloaded as they have a
unique reference number on them, please order directly with the
information center.) Every time that an adult (over 18) either
applies for a position within Scouting or changes their current role,
they will need to fill in a CRB form. The form must be filled in with a
member of the Group (i.e. GSL) or District. If they are a new applicant
they must not have unsupervised access to young people until such times
as the clearance form is received.
Awards Forms
There are many people within Scouting who put a lot of their time in
the help young people. We wish to recognise this and therefore there
are various awards that can be given. If you believe that you know
somebody who should be recognised, please contact the District
Chairman or the District Commissioner to discuss the matter. Awards can
be given to anybody within Scouting, not just Uniformed leaders.
Long Service;
There are awards for 5, 10, 15, 25 and 40 years of service to Scouting.
These are normally generated automatically by HQ, but there may be
people who have been missed out. They can still be put up for an award,
please download a form, fill in as much information as possible and
pass it onto your DC to countersign/authorise.
Cornwell Scout Badge, Gallantry or Meritorious Service Awards;
Has somebody you know in scouting preformed an extraordinary feat in
helping somebody or has carried out an act of gallantry or maybe you
know somebody who is always helping scouting. If so then they may
be suitable for one of these awards.
Thanks Badges; Thanks badges can be given by any individual
member, section, Group, or District etc to anybody, business or company
who you think has helped Scouting.
